bnx2x: fix pci device refcount leak in bnx2x_vf_is_pcie_pending()
As comment of pci_get_domain_bus_and_slot() says, it returns a pci device with refcount increment, when finish using it, the caller must decrement the reference count by calling pci_dev_put(). Call pci_dev_put() before returning from bnx2x_vf_is_pcie_pending() to avoid refcount leak.

bnx2x: Invalidate fastpath HSI version for VFs
Commit 0a6890b9b4df ("bnx2x: Utilize FW 7.13.15.0.") added validation for fastpath HSI versions for different client init which was not meant for SR-IOV VF clients, which resulted in firmware asserts when running VF clients with different fastpath HSI version.
This patch along with the new firmware support in patch #1 fixes this behavior in order to not validate fastpath HSI version for the VFs.

drivers: Remove explicit invocations of mmiowb()
mmiowb() is now implied by spin_unlock() on architectures that require it, so there is no reason to call it from driver code. This patch was generated using coccinelle:
@mmiowb@ @@ - mmiowb();
and invoked as:
$ for d in drivers include/linux/qed sound; do \ spatch --include-headers --sp-file mmiowb.cocci --dir $d --in-place; done
NOTE: mmiowb() has only ever guaranteed ordering in conjunction with spin_unlock(). However, pairing each mmiowb() removal in this patch with the corresponding call to spin_unlock() is not at all trivial, so there is a small chance that this change may regress any drivers incorrectly relying on mmiowb() to order MMIO writes between CPUs using lock-free synchronisation. If you've ended up bisecting to this commit, you can reintroduce the mmiowb() calls using wmb() instead, which should restore the old behaviour on all architectures other than some esoteric ia64 systems.

treewide: kzalloc() -> kcalloc()
The kzalloc() function has a 2-factor argument form, kcalloc(). This patch replaces cases of:
kzalloc(a * b, gfp)
with: kcalloc(a * b, gfp)
as well as handling cases of:
kzalloc(a * b * c, gfp)
with:
kzalloc(array3_size(a, b, c), gfp)
as it's slightly less ugly than:
kzalloc_array(array_size(a, b), c, gfp)
This does, however, attempt to ignore constant size factors like:
kzalloc(4 * 1024, gfp)
though any constants defined via macros get caught up in the conversion.
Any factors with a sizeof() of "unsigned char", "char", and "u8" were dropped, since they're redundant.
The Coccinelle script used for this was:
// Fix redundant parens around sizeof(). @@ type TYPE; expression THING, E; @@

bnx2x: Deprecate pci_get_bus_and_slot()
pci_get_bus_and_slot() is restrictive such that it assumes domain=0 as where a PCI device is present. This restricts the device drivers to be reused for other domain numbers.
Getting ready to remove pci_get_bus_and_slot() function in favor of pci_get_domain_bus_and_slot().
Introduce bnx2x_vf_domain() function to extract the domain information and save it to VF specific data structure.
Use the saved domain value while calling pci_get_domain_bus_and_slot().

bnx2x: Don't post statistics to malicious VFs
Once firmware indicates that a given VF is malicious and until that VF passes an FLR all bets are off - PF can't know anything is happening to the VF [since VF can't communicate anything to its PF]. But PF is currently still periodically asking device to collect statistics for the VF which might in turn fill logs by IOMMU blocking memory access done by the VF's PCI function [in the case VF has unmapped its buffers].
