gpio: tegra: Hide tegra_gpio_enable/disable()

Recent pinctrl discussions concluded that gpiolib APIs should in fact do
whatever is required to mux a GPIO onto pins, by calling pinctrl APIs if
required. This change implements this for the Tegra GPIO driver, and removes
calls to the Tegra-specific APIs from drivers and board files.

mmc: sdhci-tegra: Explicitly support Tegra30

Tegra30 differs from Tegra20 in a number of ways. This patch implements a
minimal set of differences in order to get the Cardhu board's SD slot and
eMMC working. Given the diffs between the mainline sdhci-tegra.c and our
downstream versions, I expect we'll eventually need to add many more
differences, hence the seemingly heavy-weight addition of the soc_data
structure.

* Tegra30 doesn't need to override register access to SDHCI_HOST_VERSION or
SDHCI_INT_ENABLE.

* Tegra30 needs quirk SDHCI_QUIRK_DATA_TIMEOUT_USES_SDCLK.

includecheck: delete any duplicate instances of module.h

Different tree maintainers picked up independently generated
trivial compile fixes based on linux-next testing, resulting
in some cases where a file would have got more than one addition
of module.h once everything was all merged together.

Delete any duplicates so includecheck isn't complaining about
anything related to module.h/export.h changes.

mmc: sdhci-tegra: Add 8-bit support to device tree binding.

The previous patch which implemented a DT binding for sdhci-tegra did not
allow all platform data fields to be initialized from DT. The following
were missing:

is_8bit: Implemented by this patch.

pm_flags: Not implemented yet. There are no mainline users of this field.
I'm not quite sure what it's for, and hence how to represent this
in DT; the value ends up being assigned to host->mmc->pm_caps.

While we're at it, fix the binding documentation to refer to "SD/MMC"
instead of "eSDHC", since that's the correct name; "eSDHC" was cut/paste
from the Freescale binding docs.

mmc: sdhci: change sdhci-pltfm into a module

There are a couple of problems left from the sdhci pltfm and OF
consolidation changes.

* When building more than one sdhci-pltfm based drivers in the same
image, linker will give multiple definition error on the sdhci-pltfm
helper functions. For example right now, building sdhci-of-esdhc
and sdhci-of-hlwd together is a valid combination from Kconfig view.

* With the current build method, there is error with building the
drivers as module, but module installation fails with modprobe.

The patch fixes above problems by changing sdhci-pltfm into a module.
To avoid EXPORT_SYMBOL on so many big endian IO accessors, it moves
these accessors into sdhci-pltfm.h as the 'static inline' functions.
As a result, sdhci.h needs to be included in sdhci-pltfm.h, and in
turn can be removed from individual drivers which already include
sdhci-pltfm.h.

sdhci: tegra: Add missing TMCLK for data timeout

commit b5a84ecf025a ("mmc: tegra: Add Tegra210 support")

Tegra210 and later has a separate sdmmc_legacy_tm (TMCLK) used by Tegra
SDMMC hawdware for data timeout to achive better timeout than using
SDCLK and using TMCLK is recommended.

USE_TMCLK_FOR_DATA_TIMEOUT bit in Tegra SDMMC register
SDHCI_TEGRA_VENDOR_SYS_SW_CTRL can be used to choose either TMCLK or
SDCLK for data timeout.

Default USE_TMCLK_FOR_DATA_TIMEOUT bit is set to 1 and TMCLK is used
for data timeout by Tegra SDMMC hardware and having TMCLK not enabled
is not recommended.

So, this patch adds quirk NVQUIRK_HAS_TMCLK for SoC having separate
timeout clock and keeps TMCLK enabled all the time.
