qemu-img convert (without -n) can often be replaced by a combination of _make_test_img + qemu-img convert -n. Doing so allows converting to protocols that do not allow direct file creation, such as FUSE exports. The only problem is that for formats other than qcow2 and qed (qcow1 at least), this may lead to high disk usage for some reason, so we cannot do it everywhere.
But we can do it in 028 and 089, so let us do that so they can run on FUSE exports. Also, in 028 this allows us to remove a 9-line comment that used to explain why we cannot safely filter drive-backup's image creation output.

There are many existing qcow2 images that specify a backing file but no format. This has been the source of CVEs in the past, but has become more prominent of a problem now that libvirt has switched to -blockdev. With older -drive, at least the probing was always done by qemu (so the only risk of a changed format between successive boots of a guest was if qemu was upgraded and probed differently). But with newer -blockdev, libvirt must specify a format; if libvirt guesses raw where the image was formatted, this results in data corruption visible to the guest; conversely, if libvirt guesses qcow2 where qemu was using raw, this can result in potential security holes, so modern libvirt instead refuses to use images without explicit backing format.
The change in libvirt to reject images without explicit backing format has pointed out that a number of tools have been far too reliant on probing in the past. It's time to set a better example in our own iotests of properly setting this parameter.
iotest calls to create, rebase, and convert are all impacted to some degree. It's a bit annoying that we are inconsistent on command line - while all of those accept -o backing_file=...,backing_fmt=..., the shortcuts are different: create and rebase have -b and -F, while convert has -B but no -F. (amend has no shortcuts, but the previous patch just deprecated the use of amend to change backing chains).

But there's no reason to have two separate implementations of integer to human-readable abbreviation, where one has overflow and stops at 'T', while the other avoids overflow and goes all the way to 'E'. With this patch, the output now claims 8EiB instead of -8388607T, which really is the correct rounding of largest file size supported by qemu (we could go 511 bytes larger if we used byte-accurate sizing instead of rounding up to the next sector boundary, but that wouldn't change the human-readable result).
Quite a few iotests need updates to expected output to match.

block: ignore flush requests when storage is clean
Some guests (win2008 server for example) do a lot of unnecessary flushing when underlying media has not changed. This adds additional overhead on host when calling fsync/fdatasync.
This change introduces a write generation scheme in BlockDriverState. Current write generation is checked against last flushed generation to avoid unnessesary flushes.
The problem with excessive flushing was found by a performance test which does parallel directory tree creation (from 2 processes). Results improved from 0.424 loops/sec to 0.432 loops/sec. Each loop creates 10^3 directories with 10 files in each.
This affected some blkdebug testcases that were expecting error logs from failure-injected flushes which are now skipped entirely (tests 026 071 089).
This also affects the performance of block jobs and thus BLOCK_JOB_READY events for driver-mirror and active block-commit commands now arrives faster, before QMP send successfully returns to caller (tests 141 144).

Add the bit width of every refcount entry to the format-specific information.
In contrast to lazy_refcounts and the corrupt flag, this should be always emitted, even for compat=0.10 although it does not support any refcount width other than 16 bits. This is because if a boolean is optional, one normally assumes it to be false when omitted; but if an integer is not specified, it is rather difficult to guess its value.
This new field breaks some test outputs, fix them.

qcow2_cache_flush() may fail; if one of the caches failed to be flushed successfully to disk in qcow2_close() the image should not be marked clean,

qcow2_cache_flush() may fail; if one of the caches failed to be flushed successfully to disk in qcow2_close() the image should not be marked clean, and we should emit a warning.
This breaks the (qcow2-specific) iotests 026, 071 and 089; change their output accordingly.

Just like lazy-refcounts, this field will be present iff the qcow2 compat level is 1.1 (or probably any future revision).
As expected, this breaks

As expected, this breaks some tests due to the new field present in qemu-img info output; so fix their output accordingly.

Currently, test 089 uses $QEMU_IMG info manually in order to obtain the according output. However, the iotests should generally use _img_info as this filters out more irrelevant information such as the host image size or format specific information. Therefore, test 089 should use _img_info as well.
