In update_itlb_use() the variables or_mask and and_mask are uint8_t, which means that in expressions like "and_mask << 24" the usual C arithmetic conversions will result in the shift being done as a signed int type, and so we will shift into the sign bit. For QEMU this isn't undefined behaviour because we use -fwrapv; but we can avoid it anyway by using uint32_t types for or_mask and and_mask.

Rename all DELAY_SLOT_* and GUSA_* defines to emphasize that they are included in TB_FLAGs. Add aliases for the FPSCR and SR bits that are included in TB_FLAGS, so that we don't accidentally reassign those bits.

If the get_physical_address() call fails, the SH4 get_phys_page_debug() handler returns an uninitialized address. Instead return -1, which correspond to "no page found" (see cpu_get_phys_page_debug() doc string).
This fixes a warning emitted when building with CFLAGS=-O3 (using GCC 10.2.1 20201125):

sysemu/sysemu.h is a rather unfocused dumping ground for stuff related to the system-emulator. Evidence:
* It's included widely: in my "build everything" tree, changing sysemu/sysemu.h still triggers a recompile of some 1100 out of 6600 objects (not counting tests and objects that don't depend on qemu/osdep.h, down from 5400 due to the previous two commits).
* It pulls in more than a dozen additional headers.
Split stuff related to run state management into its own header sysemu/runstate.h.
Touching sysemu/sysemu.h now recompiles some 850 objects. qemu/uuid.h also drops from 1100 to 850, and qapi/qapi-types-run-state.h from 4400 to 4200. Touching new sysemu/runstate.h recompiles some 500 objects.
Since I'm touching MAINTAINERS to add sysemu/runstate.h anyway, also add qemu/main-loop.h.
