xen: register legacy backends via xen_backend_init
It is okay to register legacy backends in the middle of xen_bus_init(). All that the registration does is record the existence of the backend in xenstore.
This makes it possible to remove them from the build without introducing undefined symbols in xen_be_init(). It also removes the need for the backend_register callback, whose only purpose is to avoid registering nonfunctional backends.

Enable PV backends with Xen/KVM emulation
This is phase 2, following on from the basic platform support which was already merged.
• Add a simple single-tenant internal XenStore implementation • Indirect Xen gnttab/evtchn/foreignmem/xenstore through operations table • Provide emulated back ends for Xen operations • Header cleanups to allow PV back ends to build without Xen itself • Enable PV back ends in emulated mode • Documentation update
Tested-by: Paul Durrant <paul@xen.org> ... on real Xen (master branch, 4.18) with a Debian guest.

hw/xen: Pass grant ref to gnttab unmap operation
The previous commit introduced redirectable gnttab operations fairly much like-for-like, with the exception of the extra arguments to the ->open() call which were always NULL/0 anyway.
This *changes* the arguments to the ->unmap() operation to include the original ref# that was mapped. Under real Xen it isn't necessary; all we need to do from QEMU is munmap(), then the kernel will release the grant, and Xen does the tracking/refcounting for the guest.
When we have emulated grant tables though, we need to do all that for ourselves. So let's have the back ends keep track of what they mapped and pass it in to the ->unmap() method for us.

xen: Fix and improve handling of device_add usb-host errors
usbback_portid_add() leaks the error when qdev_device_add() fails. Fix that. While there, use the error to improve the error message.
The qemu_opts_from_qdict() similarly leaks on failure. But any failure there is a programming error. Pass &error_abort.

hw/usb/xen-usb.c: Pass struct usbback_req* to usbback_packet_complete()
The function usbback_packet_complete() currently takes a USBPacket*, which must be a pointer to the packet field within a struct usbback_req; the function uses container_of() to get the struct usbback_req* given the USBPacket*.
This is unnecessarily confusing (and in particular it confuses the Coverity Scan analysis, resulting in the false positive CID 1421919 where it thinks that we write off the end of the structure). Since both callsites already have the pointer to the struct usbback_req, just pass that in directly.

Include qemu/main-loop.h less
In my "build everything" tree, changing qemu/main-loop.h triggers a recompile of some 5600 out of 6600 objects (not counting tests and objects that don't depend on qemu/osdep.h). It includes block/aio.h, which in turn includes qemu/event_notifier.h, qemu/notify.h, qemu/processor.h, qemu/qsp.h, qemu/queue.h, qemu/thread-posix.h, qemu/thread.h, qemu/timer.h, and a few more.
Include qemu/main-loop.h only where it's needed. Touching it now recompiles only some 1700 objects. For block/aio.h and qemu/event_notifier.h, these numbers drop from 5600 to 2800. For the others, they shrink only slightly.

xen: Import other xen/io/*.h
A Xen public header have been imported into QEMU (by f65eadb639 "xen: import ring.h from xen"), but there are other header that depends on ring.h which come from the system when building QEMU.
This patch resolves the issue of having headers from the system importing a different copie of ring.h.
This patch is prompt by the build issue described in the previous patch: 'Revert xen/io/ring.h of "Clean up a few header guard symbols"'
ring.h and the new imported headers are moved to "include/hw/xen/interface" as those describe interfaces with a guest.
The imported headers are cleaned up a bit while importing them: some part of the file that QEMU doesn't use are removed (description of how to make hypercall in grant_table.h have been removed).
Other cleanup: - xen-mapcache.c and xen-legacy-backend.c don't need grant_table.h. - xenfb.c doesn't need event_channel.h.

xen: re-name XenDevice to XenLegacyDevice...
...and xen_backend.h to xen-legacy-backend.h
Rather than attempting to convert the existing backend infrastructure to be QOM compliant (which would be hard to do in an incremental fashion), subsequent patches will introduce a completely new framework for Xen PV backends. Hence it is necessary to re-name parts of existing code to avoid name clashes. The re-named 'legacy' infrastructure will be removed once all backends have been ported to the new framework.
This patch is purely cosmetic. No functional change.

qemu/queue.h: leave head structs anonymous unless necessary
Most list head structs need not be given a name. In most cases the name is given just in case one is going to use QTAILQ_LAST, QTAILQ_PREV or reverse iteration, but this does not apply to lists of other kinds, and even for QTAILQ in practice this is only rarely needed. In addition, we will soon reimplement those macros completely so that they do not need a name for the head struct. So clean up everything, not giving a name except in the rare case where it is necessary.
