clock: Add ClockEvent parameter to callbacks

The Clock framework allows users to specify a callback which is
called after the clock's period has been updated. Some users need to
also have a callback which is called before the clock period is
updated.

As the first step in adding support for notifying Clock users on
pre-update events, add an argument to the ClockCallback to specify
what event is being notified, and add an argument to the various
functions for registering a callback to specify which events are
of interest to that callback.

Note that the documentation update renders correct the previously
incorrect claim in 'Adding a new clock' that callbacks "will be
explained in a following section".

with this clock;

As the first step in converting the CMSDK_APB_DUALTIMER device to the
Clock framework, add a Clock input. For the moment we do nothing
with this clock; we will change the behaviour from using the pclk-frq
property to using the Clock once all the users of this device have
been converted to wire up the Clock.

We take the opportunity to correct the name of the clock input to
match the hardware -- the dual timer names the clock which drives the
timers TIMCLK. (It does also have a 'pclk' input, which is used only
for the register and APB bus logic; on the SSE-200 these clocks are
both connected together.)

This is a migration compatibility break for machines mps2-an385,
mps2-an386, mps2-an500, mps2-an511, mps2-an505, mps2-an521, musca-a,
musca-b1.

mechanism for calling back into the device model using the
ptimer when the timer has

ptimer: Rename ptimer_init() to ptimer_init_with_bh()

Currently the ptimer design uses a QEMU bottom-half as its
mechanism for calling back into the device model using the
ptimer when the timer has expired. Unfortunately this design
is fatally flawed, because it means that there is a lag
between the ptimer updating its own state and the device
callback function updating device state, and guest accesses
to device registers between the two can return inconsistent
device state.

We want to replace the bottom-half design with one where
the guest device's callback is called either immediately
(when the ptimer triggers by timeout) or when the device
model code closes a transaction-begin/end section (when the
ptimer triggers because the device model changed the
ptimer's count value or other state). As the first step,
rename ptimer_init() to ptimer_init_with_bh(), to free up
the ptimer_init() name for the new API. We can then convert
all the ptimer users away from ptimer_init_with_bh() before
removing it entirely.

git grep -l ptimer_init | xargs sed -i -e 's/ptimer_init/ptimer_init_with_bh/'
and three overlong lines folded by hand.)

In my "build everything" tree, changing hw/qdev-properties.h triggers
a recompile of some 2700 out of 6600 objects (not counting tests and
objects that don't depend on qemu/osdep.h).

Many places including hw/qdev-properties.h (directly or via hw/qdev.h)
actually need only hw/qdev-core.h. Include hw/qdev-core.h there
instead.

hw/qdev.h is actually pointless: all it does is include hw/qdev-core.h
and hw/qdev-properties.h, which in turn includes hw/qdev-core.h.
Replace the remaining uses of hw/qdev.h by hw/qdev-properties.h.

While there, delete a few superfluous inclusions of hw/qdev-core.h.

Touching hw/qdev-properties.h now recompiles some 1200 objects.
