hw/scsi/esp-pci: set DMA_STAT_BCMBLT when BLAST command issued
Even though the BLAST command isn't fully implemented in QEMU, the DMA_STAT_BCMBLT bit should be set after the command has been issued to indicate that the command has completed.
This fixes an issue with the DC390 DOS driver which issues the BLAST command as part of its normal error recovery routine at startup, and otherwise sits in a tight loop waiting for DMA_STAT_BCMBLT to be set before continuing.

hw/scsi/esp-pci: synchronise setting of DMA_STAT_DONE with ESP completion interrupt
The setting of DMA_STAT_DONE at the end of a DMA transfer can be configured to generate an interrupt, however the Linux driver manually checks for DMA_STAT_DONE being set and if it is, considers that a DMA transfer has completed.
If DMA_STAT_DONE is set but the ESP device isn't indicating an interrupt then the Linux driver considers this to be a spurious interrupt. However this can occur in QEMU as there is a delay between the end of DMA transfer where DMA_STAT_DONE is set, and the ESP device raising its completion interrupt.
This appears to be an incorrect assumption in the Linux driver as the ESP and PCI DMA interrupt sources are separate (and may not be raised exactly together), however we can work around this by synchronising the setting of DMA_STAT_DONE at the end of a DMA transfer with the ESP completion interrupt.
In conjunction with the previous commit Linux is now able to correctly boot from an am53c974 PCI SCSI device on the hppa C3700 machine without emitting "iget: checksum invalid" and "Spurious irq, sreg=10" errors.

hw/scsi/esp-pci: generate PCI interrupt from separate ESP and PCI sources
The am53c974/dc390 PCI interrupt has two separate sources: the first is from the internal ESP device, and the second is from the PCI DMA transfer logic.
Update the ESP interrupt handler so that it sets DMA_STAT_SCSIINT rather than driving the PCI IRQ directly, and introduce a new esp_pci_update_irq() function to generate the correct PCI IRQ level. In particular this fixes spurious interrupts being generated by setting DMA_STAT_DONE at the end of a transfer if DMA_CMD_INTE_D isn't set in the DMA_CMD register.

hw/scsi/esp-pci: use correct address register for PCI DMA transfers
The current code in esp_pci_dma_memory_rw() sets the DMA address to the value of the DMA_SPA (Starting Physical Address) register which is incorrect: this means that for each callback from the SCSI layer the DMA address is set back to the starting address.
In the case where only a single SCSI callback occurs (currently for transfer lengths < 128kB) this works fine, however for larger transfers the DMA address wraps back to the initial starting address, corrupting the buffer holding the data transferred to the guest.
Fix esp_pci_dma_memory_rw() to use the DMA_WAC (Working Address Counter) for the DMA address which is correctly incremented across multiple SCSI layer transfers.

include/hw/pci: Split pci_device.h off pci.h
PCIDeviceClass and PCIDevice are defined in pci.h. Many users of the header don't actually need them. Similar structs live in their own headers: PCIBusClass and PCIBus in pci_bus.h, PCIBridge in pci_bridge.h, PCIHostBridgeClass and PCIHostState in pci_host.h, PCIExpressHost in pcie_host.h, and PCIERootPortClass, PCIEPort, and PCIESlot in pcie_port.h.
Move PCIDeviceClass and PCIDeviceClass to new pci_device.h, along with the code that needs them. Adjust include directives.
This also enables the next commit.

scsi: Replace scsi_bus_new() with scsi_bus_init(), scsi_bus_init_named()
The function scsi_bus_new() creates a new SCSI bus; callers can either pass in a name argument to specify the name of the new bus, or they can pass in NULL to allow the bus to be given an automatically generated unique name. Almost all callers want to use the autogenerated name; the only exception is the virtio-scsi device.
Taking a name argument that should almost always be NULL is an easy-to-misuse API design -- it encourages callers to think perhaps they should pass in some standard name like "scsi" or "scsi-bus". We don't do this anywhere for SCSI, but we do (incorrectly) do it for other bus types such as i2c.
The function name also implies that it will return a newly allocated object, when it in fact does in-place allocation. We more commonly name such functions foo_init(), with foo_new() being the allocate-and-return variant.
Replace all the scsi_bus_new() callsites with either: * scsi_bus_init() for the usual case where the caller wants an autogenerated bus name * scsi_bus_init_named() for the rare case where the caller needs to specify the bus name
and document that for the _named() version it's then the caller's responsibility to think about uniqueness of bus names.

esp: fix setting of ESPState mig_version_id when launching QEMU with -S option
If QEMU is launched with the -S option then the ESPState mig_version_id property is left unset due to the ordering of the VMState fields in the VMStateDescription for sysbusespscsi and pciespscsi. If the VM is migrated and restored in this stopped state, the version tests in the vmstate_esp VMStateDescription and esp_post_load() become confused causing the migration to fail.
Fix the ordering problem by moving the setting of mig_version_id to a common esp_pre_save() function which is invoked first by both sysbusespscsi and pciespscsi rather than at the point where ESPState is itself serialised into the migration stream.

esp: add vmstate_esp version to embedded ESPState
The QOM object representing ESPState is currently embedded within both the SYSBUS_ESP and PCI_ESP devices with migration state handled by embedding vmstate_esp within each device using VMSTATE_STRUCT.
Since the vmstate_esp fields are embedded directly within the migration stream, the incoming vmstate_esp version_id is lost. The only version information available is that from vmstate_sysbus_esp_scsi and vmstate_esp_pci_scsi, but those versions represent their respective devices and not that of the underlying ESPState.
Resolve this by adding a new version-dependent field in vmstate_sysbus_esp_scsi and vmstate_esp_pci_scsi which stores the vmstate_esp version_id field within ESPState to be used to allow migration from older QEMU versions.
Finally bump the vmstate_esp version to 5 to cover the upcoming ESPState changes within this patch series.
