remove a call to device_class_set_parent_reset(), replacing it with
the three-ph

hw/s390/virtio-ccw: Convert to three-phase reset

Convert the virtio-ccw code to three-phase reset. This allows us to
remove a call to device_class_set_parent_reset(), replacing it with
the three-phase equivalent resettable_class_set_parent_phases().
Removing all the device_class_set_parent_reset() uses will allow us
to remove some of the glue code that interworks between three-phase
and legacy reset.

This is a simple conversion, with no behavioural changes.

trigger a failed assertion down the call path of
virtio_lduw_phys_cached().

Fix this by calling virtio_init_region_cache() after
virtio_queue_set_num() if we are not already calling
virtio_queue_set_rings(). In the legacy path this is already done by
virtio_queue_update_rings().

and IRB.ESW. But for passthrough (vfio-ccw) subchannels,
this data isn't populated in the first place, so enabling
those flags leads to unexpected behavior if the guest tries to
process the sense data (zeros) in the IRB.ECW.

Let's add a subchannel callback that builds these portions of
the IRB, and move the existing code into a routine for those
virtual subchannels. The passthrough subchannels will be able
to piggy-back onto this later.

hw: Do not include hw/sysbus.h if it is not necessary

Many files include hw/sysbus.h without needing it. Remove the superfluous
include statements.
