hw/riscv: add RISC-V IOMMU base emulation

The RISC-V IOMMU specification is now ratified as-per the RISC-V
international process. The latest frozen specifcation can be found at:

https://github.com/riscv-non-isa/riscv-iommu/releases/download/v1.0/riscv-iommu.pdf

Add the foundation of the device emulation for RISC-V IOMMU. It includes
support for s-stage (sv32, sv39, sv48, sv57 caps) and g-stage (sv32x4,
sv39x4, sv48x4, sv57x4 caps).

Other capabilities like ATS and DBG support will be added incrementally
in the next patches.

trace-events: Fix attribution of trace points to source

Some trace points are attributed to the wrong source file. Happens
when we neglect to update trace-events for code motion, or add events
in the wrong place, or misspell the file name.

Clean up with help of scripts/cleanup-trace-events.pl. Funnies
requiring manual post-processing:

* accel/tcg/cputlb.c trace points are in trace-events.

* block.c and blockdev.c trace points are in block/trace-events.

* hw/block/nvme.c uses the preprocessor to hide its trace point use
from cleanup-trace-events.pl.

* hw/tpm/tpm_spapr.c uses pseudo trace point tpm_spapr_show_buffer to
guard debug code.

* include/hw/xen/xen_common.h trace points are in hw/xen/trace-events.

* linux-user/trace-events abbreviates a tedious list of filenames to
*/signal.c.

* net/colo-compare and net/filter-rewriter.c use pseudo trace points
colo_compare_miscompare and colo_filter_rewriter_debug to guard
debug code.

Merge remote-tracking branch 'remotes/palmer/tags/riscv-for-master-4.1-sf0' into staging

RISC-V Patches for the 4.1 Soft Freeze, Part 1

This tag contains a handful of patches that I'd like to target for 4.1:

* An emulation for SiFive's GPIO device.
* A fix to disallow sfence.vma from userspace.
* Additional decodetree cleanups that should have no functional impact.
* C extension emulation fidelity fixes that were noticed as part of that
cleanup process.
* A new "spike" target, along with the deprecation of a handful of old
targets and CPUs.
* Some initial infastructure related to the hypervisor extension.
* An emulation fidelity fix that prevents prevents arbitrary bits in the
SIP CSR from being set.
* A small performance improvement that avoids excessive TLB flushing
when the ASID does not change.

This time I've used a new testing workflow: I've tested on both 32-bit
and 64-bit builds of OpenEmbedded, via the default OpenSBI-based boot
flow.
