ui+display: rename is_buffer_shared() -> surface_is_allocated()
Boolean return value is reversed, to align with QEMU_ALLOCATED_FLAG, so all callers must be adapted. Also rename share_surface variable in vga_draw_graphic() to reduce confusion.

hw/xen: Register framebuffer backend via xen_backend_init()
Align the framebuffer backend with the other legacy ones, register it via xen_backend_init() when '-vga xenfb' is used. It is safe because MODULE_INIT_XEN_BACKEND is called in xen_bus_realize(), long after CLI processing initialized the vga_interface_type variable.

xen: register legacy backends via xen_backend_init
It is okay to register legacy backends in the middle of xen_bus_init(). All that the registration does is record the existence of the backend in xenstore.
This makes it possible to remove them from the build without introducing undefined symbols in xen_be_init(). It also removes the need for the backend_register callback, whose only purpose is to avoid registering nonfunctional backends.

Enable PV backends with Xen/KVM emulation
This is phase 2, following on from the basic platform support which was already merged.
• Add a simple single-tenant internal XenStore implementation • Indirect Xen gnttab/evtchn/foreignmem/xenstore through operations table • Provide emulated back ends for Xen operations • Header cleanups to allow PV back ends to build without Xen itself • Enable PV back ends in emulated mode • Documentation update
Tested-by: Paul Durrant <paul@xen.org> ... on real Xen (master branch, 4.18) with a Debian guest.

hw/xen: Use XEN_PAGE_SIZE in PV backend drivers
XC_PAGE_SIZE comes from the actual Xen libraries, while XEN_PAGE_SIZE is provided by QEMU itself in xen_backend_ops.h. For backends which may be built for emulation mode, use the latter.

ui: Deliver refresh rate via QemuUIInfo
This change adds a new member, refresh_rate to QemuUIInfo in include/ui/console.h. It represents the refresh rate of the physical display backend, and it is more appropriate than GUI update interval as the refresh rate which the emulated device reports: - sdl may set GUI update interval shorter than the refresh rate of the physical display to respond to user-generated events. - sdl and vnc aggressively changes GUI update interval, but a guests is typically not designed to respond to frequent refresh rate changes, or frequent "display mode" changes in general. The frequency of refresh rate changes of the physical display backend matches better to the guest's expectation.
QemuUIInfo also has other members representing "display mode", which makes it suitable for refresh rate representation. It has a throttling of update notifications, and prevents frequent changes of the display mode.

Use g_new() & friends where that makes obvious sense
g_new(T, n) is neater than g_malloc(sizeof(T) * n). It's also safer, for two reasons. One, it catches multiplication overflowing size_t. Two, it returns T * rather than void *, which lets the compiler catch more type errors.
This commit only touches allocations with size arguments of the form sizeof(T).

Include hw/hw.h exactly where needed
In my "build everything" tree, changing hw/hw.h triggers a recompile of some 2600 out of 6600 objects (not counting tests and objects that don't depend on qemu/osdep.h).
The previous commits have left only the declaration of hw_error() in hw/hw.h. This permits dropping most of its inclusions. Touching it now recompiles less than 200 objects.

xen: Import other xen/io/*.h
A Xen public header have been imported into QEMU (by f65eadb639 "xen: import ring.h from xen"), but there are other header that depends on ring.h which come from the system when building QEMU.
This patch resolves the issue of having headers from the system importing a different copie of ring.h.
This patch is prompt by the build issue described in the previous patch: 'Revert xen/io/ring.h of "Clean up a few header guard symbols"'
ring.h and the new imported headers are moved to "include/hw/xen/interface" as those describe interfaces with a guest.
The imported headers are cleaned up a bit while importing them: some part of the file that QEMU doesn't use are removed (description of how to make hypercall in grant_table.h have been removed).
Other cleanup: - xen-mapcache.c and xen-legacy-backend.c don't need grant_table.h. - xenfb.c doesn't need event_channel.h.

xen: re-name XenDevice to XenLegacyDevice...
...and xen_backend.h to xen-legacy-backend.h
Rather than attempting to convert the existing backend infrastructure to be QOM compliant (which would be hard to do in an incremental fashion), subsequent patches will introduce a completely new framework for Xen PV backends. Hence it is necessary to re-name parts of existing code to avoid name clashes. The re-named 'legacy' infrastructure will be removed once all backends have been ported to the new framework.
This patch is purely cosmetic. No functional change.
