xfs_attr3_leaf_split propagates the need for an extra btree split as -ENOSPC to it's only caller, but the same return value can also be returned from xfs_da_grow_inode when it fails to find free space.
Distinguish the two cases by returning 1 for the extra split case instead of overloading -ENOSPC.
This can be triggered relatively easily with the pending realtime group support and a file system with a lot of small zones that use metadata space on the main device. In this case every about 5-10th run of xfs/538 runs into the following assert:
ASSERT(oldblk->magic == XFS_ATTR_LEAF_MAGIC);
in xfs_attr3_leaf_split caused by an allocation failure. Note that the allocation failure is caused by another bug that will be fixed subsequently, but this commit at least sorts out the error handling.

xfs_bmapi_write can return 0 without actually returning a mapping in mval in two different cases:
1) when there is absolutely no space available to do an allocation 2) when converting delalloc space, and the allocation is so small that it only covers parts of the delalloc extent before the range requested by the caller
Callers at best can handle one of these cases, but in many cases can't cope with either one. Switch xfs_bmapi_write to always return a mapping or return an error code instead. For case 1) above ENOSPC is the obvious choice which is very much what the callers expect anyway. For case 2) there is no really good error code, so pick a funky one from the SysV streams portfolio.
This fixes the reproducer here:

which uses reserved blocks to create file systems that are gravely out of space and thus cause at least xfs_file_alloc_space to hang and trigger the lack of ENOSPC handling in xfs_dquot_disk_alloc.
Note that this patch does not actually make any caller but xfs_alloc_file_space deal intelligently with case 2) above.

xfs_da3_swap_lastblock() copy the last block content to the dead block, but do not update the metadata in it. We need update some metadata for some kinds of type block, such as dir3 leafn block records its blkno, we shall update it to the dead block blkno. Otherwise, before write the xfs_buf to disk, the verify_write() will fail in blk_hdr->blkno != xfs_buf->b_bn, then xfs will be shutdown.

Take a look at the for-loop in xfs_da_grow_inode_int: ====== for(){ nmap = min(XFS_BMAP_MAX_NMAP, count); ... error = xfs_bmapi_write(...,&mapp[mapi], &nmap);//(..., $1, $2) ... mapi += nmap; } ===== where $1 stands for the start address of the array, while $2 is used to indicate the size of the array.
The array $1 will advance by $nmap in each iteration after the allocation of extents. But the size $2 still remains unchanged, which is determined by min(XFS_BMAP_MAX_NMAP, count).
It seems that it has forgotten to trim the mapp array after each iteration, so change it.
