Previously, the NVMe/TCP host driver did not handle the C2HTermReq PDU,
instead printing "unsupported pdu type (3)" when received. This patch adds
support for processing the C2HTermReq PDU, allowing the driver
to print the Fatal Error Status field.

The WARN is observed when the blktests test case nvme/014 is repeated
with tcp transport. It is rare, and 200 times repeat is required to
recreate in some test environments.

To avoid the WARN, check the NVME_TCP_Q_LIVE flag before locking
queue->queue_lock. The flag is cleared long time before the lock gets
destroyed.

A different CPU may be setting the ctrl->state value, so ensure proper
barriers to prevent optimizing to a stale state. Normally it isn't a
problem to observe the wrong state as it is merely advisory to take a
quicker path during initialization and error recovery, but seeing an old
state can report unexpected ENETRESET errors when a reset request was in
fact successful.

shifting the iommufd_object_deref_user() into iommuf

Required for following patches.

Resolve merge conflict by using the hunk from the for-next branch and
shifting the iommufd_object_deref_user() into iommufd_hw_pagetable_put()
