block: initialize integrity buffer to zero before writing it to media
[ Upstream commit 899ee2c3829c5ac14bfc7d3c4a5846c0b709b78f ]
Metadata added by bio_integrity_prep is using plain kmalloc, which leads to random kernel memory being written media. For PI metadata this is limited to the app tag that isn't used by kernel generated metadata, but for non-PI metadata the entire buffer leaks kernel memory.
Fix this by adding the __GFP_ZERO flag to allocations for writes.

bio-integrity: create multi-page bvecs in bio_integrity_add_page()
In general, the bvec data structure consists of one for physically continuous pages. But, in the bvec configuration for bip, physically continuous integrity pages are composed of each bvec.
Allow bio_integrity_add_page() to create multi-page bvecs, just like the bio payloads. This simplifies adding larger payloads, and fixes support for non-tiny workloads with nvme, which stopped using scatterlist for metadata a while ago.

bio-integrity: update the payload size in bio_integrity_add_page()
Previously, the bip's bi_size has been set before an integrity pages were added. If a problem occurs in the process of adding pages for bip, the bi_size mismatch problem must be dealt with.
When the page is successfully added to bvec, the bi_size is updated.
The parts affected by the change were also contained in this commit.

block: bio-integrity: Copy flags when bio_integrity_payload is cloned
Make sure to copy the flags when a bio_integrity_payload is cloned. Otherwise per-I/O properties such as IP checksum flag will not be passed down to the HBA driver.
