xen/events: reduce externally visible helper functions
[ Upstream commit 686464514fbebb6c8de4415238319e414c3500a4 ]
get_evtchn_to_irq() has only one external user while irq_from_evtchn() provides the same functionality and is exported for a wider user base. Modify the only external user of get_evtchn_to_irq() to use irq_from_evtchn() instead and make get_evtchn_to_irq() static.
evtchn_from_irq() and irq_from_virq() have a single external user and can easily be combined to a new helper irq_evtchn_from_virq() allowing to drop irq_from_virq() and to make evtchn_from_irq() static.

xen: simplify evtchn_do_upcall() call maze
There are several functions involved for performing the functionality of evtchn_do_upcall():
- __xen_evtchn_do_upcall() doing the real work - xen_hvm_evtchn_do_upcall() just being a wrapper for __xen_evtchn_do_upcall(), exposed for external callers - xen_evtchn_do_upcall() calling __xen_evtchn_do_upcall(), too, but without any user
Simplify this maze by:
- removing the unused xen_evtchn_do_upcall() - removing xen_hvm_evtchn_do_upcall() as the only left caller of __xen_evtchn_do_upcall(), while renaming __xen_evtchn_do_upcall() to xen_evtchn_do_upcall()

xen/evtchn: Introduce new IOCTL to bind static evtchn
Xen 4.17 supports the creation of static evtchns. To allow user space application to bind static evtchns introduce new ioctl "IOCTL_EVTCHN_BIND_STATIC". Existing IOCTL doing more than binding that’s why we need to introduce the new IOCTL to only bind the static event channels.
Static evtchns to be available for use during the lifetime of the guest. When the application exits, __unbind_from_irq() ends up being called from release() file operations because of that static evtchns are getting closed. To avoid closing the static event channel, add the new bool variable "is_static" in "struct irq_info" to mark the event channel static when creating the event channel to avoid closing the static evtchn.
Also, take this opportunity to remove the open-coded version of the evtchn close in drivers/xen/evtchn.c file and use xen_evtchn_close().

xen: Allow platform PCI interrupt to be shared
When we don't use the per-CPU vector callback, we ask Xen to deliver event channel interrupts as INTx on the PCI platform device. As such, it can be shared with INTx on other PCI devices.
Set IRQF_SHARED, and make it return IRQ_HANDLED or IRQ_NONE according to whether the evtchn_upcall_pending flag was actually set. Now I can share the interrupt:
11: 82 0 IO-APIC 11-fasteoi xen-platform-pci, ens4
Drop the IRQF_TRIGGER_RISING. It has no effect when the IRQ is shared, and besides, the only effect it was having even beforehand was to trigger a debug message in both I/OAPIC and legacy PIC cases:
[ 0.915441] genirq: No set_type function for IRQ 11 (IO-APIC) [ 0.951939] genirq: No set_type function for IRQ 11 (XT-PIC)

xen/console: harden hvc_xen against event channel storms
The Xen console driver is still vulnerable for an attack via excessive number of events sent by the backend. Fix that by using a lateeoi event channel.
For the normal domU initial console this requires the introduction of bind_evtchn_to_irq_lateeoi() as there is no xenbus device available at the time the event channel is bound to the irq.
As the decision whether an interrupt was spurious or not requires to test for bytes having been read from the backend, move sending the event into the if statement, as sending an event without having found any bytes to be read is making no sense at all.

xen/events: add a new "late EOI" evtchn framework
In order to avoid tight event channel related IRQ loops add a new framework of "late EOI" handling: the IRQ the event channel is bound to will be masked until the event has been handled and the related driver is capable to handle another event. The driver is responsible for unmasking the event channel via the new function xen_irq_lateeoi().
This is similar to binding an event channel to a threaded IRQ, but without having to structure the driver accordingly.
In order to support a future special handling in case a rogue guest is sending lots of unsolicited events, add a flag to xen_irq_lateeoi() which can be set by the caller to indicate the event was a spurious one.
This is part of XSA-332.

Make the PV specific code in arch/x86/entry/entry_*.S dependent on CONFIG_XEN_PV instead of CONFIG_XEN.
The HVM specific code should depend on CONFIG_XEN_PVHVM.
While at it reformat the Makefile to make it more readable.

License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0' SPDX license identifier. The SPDX identifier is a legally binding shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of the use cases: - file had no licensing information it it. - file was a */uapi/* one with no licensing information in it, - file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases where non-standard license headers were used, and references to license had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to a file was done in a spreadsheet of side by side results from of the output of two independent scanners (ScanCode & Windriver) producing SPDX tag:value files created by Philippe Ombredanne. Philippe prepared the base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files assessed. Kate Stewart did a file by file comparison of the scanner results in the spreadsheet to determine which SPDX license identifier(s) to be applied to the file. She confirmed any determination that was not immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was: - Files considered eligible had to be source code files. - Make and config files were included as candidates if they contained >5 lines of source - File already had some variant of a license header in it (even if <5 lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license identifiers to apply.
- when both scanners couldn't find any license traces, file was considered to have no license information in it, and the top level COPYING file license applied.
For non */uapi/* files that summary was:
SPDX license identifier # files ---------------------------------------------------|------- GPL-2.0 11139
and resulted in the first patch in this series.
If that file was a */uapi/* path one, it was "GPL-2.0 WITH Linux-syscall-note" otherwise it was "GPL-2.0". Results of that was:
SPDX license identifier # files ---------------------------------------------------|------- GPL-2.0 WITH Linux-syscall-note 930
and resulted in the second patch in this series.
- if a file had some form of licensing information in it, and was one of the */uapi/* ones, it was denoted with the Linux-syscall-note if any GPL family license was found in the file or had no licensing in it (per prior point). Results summary:
SPDX license identifier # files ---------------------------------------------------|------ GPL-2.0 WITH Linux-syscall-note 270 GPL-2.0+ WITH Linux-syscall-note 169 ((GPL-2.0 WITH Linux-syscall-note) OR BSD-2-Clause) 21 ((GPL-2.0 WITH Linux-syscall-note) OR BSD-3-Clause) 17 LGPL-2.1+ WITH Linux-syscall-note 15 GPL-1.0+ WITH Linux-syscall-note 14 ((GPL-2.0+ WITH Linux-syscall-note) OR BSD-3-Clause) 5 LGPL-2.0+ WITH Linux-syscall-note 4 LGPL-2.1 WITH Linux-syscall-note 3 ((GPL-2.0 WITH Linux-syscall-note) OR MIT) 3 ((GPL-2.0 WITH Linux-syscall-note) AND MIT) 1
and that resulted in the third patch in this series.
- when the two scanners agreed on the detected license(s), that became the concluded license(s).
- when there was disagreement between the two scanners (one detected a license but the other didn't, or they both detected different licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file resulted in a clear resolution of the license that should apply (and which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier, the file was flagged for further research and to be revisited later in time.
In total, over 70 hours of logged manual review was done on the spreadsheet to determine the SPDX license identifiers to apply to the source files by Kate, Philippe, Thomas and, in some cases, confirmation by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from FOSSology, and compared selected files where the other two scanners disagreed against that SPDX file, to see if there was new insights. The Windriver scanner is based on an older version of FOSSology in part, so they are related.
Thomas did random spot checks in about 500 files from the spreadsheets for the uapi headers and agreed with SPDX license identifier in the files he inspected. For the non-uapi files Thomas did random spot checks in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have copy/paste license identifier errors, and have been fixed to reflect the correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual inspection and review of the 12,461 patched files from the initial patch version early this week with: - a full scancode scan run, collecting the matched texts, detected license ids and scores - reviewing anything where there was a license detected (about 500+ files) to ensure that the applied SPDX license was correct - reviewing anything where there was no detection but the patch license was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This worksheet was then exported into 3 different .csv files for the different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to parse the csv files and add the proper SPDX tag to the file, in the format that the file expected. This script was further refined by Greg based on the output to detect more types of files automatically and to distinguish between header and source .c files (which need different comment types.) Finally Greg ran the script using the .csv files to generate the patches.

xen-evtchn: Bind dyn evtchn:qemu-dm interrupt to next online VCPU
A HVM domian booting generates around 200K (evtchn:qemu-dm xen-dyn) interrupts,in a short period of time. All these evtchn:qemu-dm are bound to VCPU 0, until irqbalance sees these IRQ and moves it to a different VCPU. In one configuration, irqbalance runs every 10 seconds, which means irqbalance doesn't get to see these burst of interrupts and doesn't re-balance interrupts most of the time, making all evtchn:qemu-dm to be processed by VCPU0. This cause VCPU0 to spend most of time processing hardirq and very little time on softirq. Moreover, if dom0 kernel PREEMPTION is disabled, VCPU0 never runs watchdog (process context), triggering a softlockup detection code to panic.
Binding evtchn:qemu-dm to next online VCPU, will spread hardirq processing evenly across different CPU. Later, irqbalance will try to balance evtchn:qemu-dm, if required.

xen/events: Support event channel rebind on ARM
Currently, the event channel rebind code is gated with the presence of the vector callback.
The virtual interrupt controller on ARM has the concept of per-CPU interrupt (PPI) which allow us to support per-VCPU event channel. Therefore there is no need of vector callback for ARM.
Xen is already using a free PPI to notify the guest VCPU of an event. Furthermore, the xen code initialization in Linux (see arch/arm/xen/enlighten.c) is requesting correctly a per-CPU IRQ.
Introduce new helper xen_support_evtchn_rebind to allow architecture decide whether rebind an event is support or not. It will always return true on ARM and keep the same behavior on x86.
This is also allow us to drop the usage of xen_have_vector_callback entirely in the ARM code.

xen/events: don't bind non-percpu VIRQs with percpu chip
A non-percpu VIRQ (e.g., VIRQ_CONSOLE) may be freed on a different VCPU than it is bound to. This can result in a race between handle_percpu_irq() and removing the action in __free_irq() because handle_percpu_irq() does not take desc->lock. The interrupt handler sees a NULL action and oopses.
Only use the percpu chip/handler for per-CPU VIRQs (like VIRQ_TIMER).
