io-mapping: don't disable preempt on RT in io_mapping_map_atomic_wc().
io_mapping_map_atomic_wc() disables preemption and pagefaults for historical reasons. The conversion to io_mapping_map_local_wc(), which only disables migration, cannot be done wholesale because quite some call sites need to be updated to accommodate with the changed semantics.
On PREEMPT_RT enabled kernels the io_mapping_map_atomic_wc() semantics are problematic due to the implicit disabling of preemption which makes it impossible to acquire 'sleeping' spinlocks within the mapped atomic sections.
PREEMPT_RT replaces the preempt_disable() with a migrate_disable() for more than a decade. It could be argued that this is a justification to do this unconditionally, but PREEMPT_RT covers only a limited number of architectures and it disables some functionality which limits the coverage further.
Limit the replacement to PREEMPT_RT for now. This is also done kmap_atomic().

The fallback was introduced in commit 80c33624e472 ("io-mapping: Fixup for different names of writecombine") to fix the build on microblaze.
5 years later, it seems all archs now provide a pgprot_writecombine(), so just remove the other possible fallbacks. For microblaze, pgprot_writecombine() is available since commit 97ccedd793ac ("microblaze: Provide pgprot_device/writecombine macros for nommu").
This is build-tested on microblaze with a hack to always build mm/io-mapping.o and without DIYing on an x86-only macro (_PAGE_CACHE_MASK)

The !ATOMIC_IOMAP version of io_maping_init_wc will always return success, even when the ioremap fails.
Since the ATOMIC_IOMAP version returns NULL when the init fails, and callers check for a NULL return on error this is unexpected.
During a device probe, where the ioremap failed, a crash can look like this:

Currently, we only allocate a structure to hold metadata if we need to allocate an ioremap for every access, such as on x86-32. However, it would be useful to store basic information about the io-mapping, such as its page protection, on all platforms.

If a header file is making use of BUG, BUG_ON, BUILD_BUG_ON, or any other BUG variant in a static inline (i.e. not in a #define) then that header really should be including <linux/bug.h> and not just expecting it to be implicitly present.
We can make this change risk-free, since if the files using these headers didn't have exposure to linux/bug.h already, they would have been causing compile failures/warnings.

For the !HAVE_ATOMIC_IOMAP case the stub functions did not call pagefault_disable/_enable. The i915 driver relies on the map actually being atomic, otherwise it can deadlock with it's own pagefault handler in the gtt pwrite fastpath.
This is exercised by gem_mmap_gtt from the intel-gpu-toosl gem testsuite.

and in general can stop worrying about what context we're in and what kmap slots might be appropriate for that.
The downside is that FRV kmap_atomic() gets more expensive.
For now we use a CPP trick suggested by Andrew:

to avoid having to touch all kmap_atomic() users in a single patch.
