usb: ehci-mv: Convert to platform remove callback returning void
The .remove() callback for a platform driver returns an int which makes many driver authors wrongly assume it's possible to do error handling by returning an error code. However the value returned is ignored (apart from emitting a warning) and this typically results in resource leaks. To improve here there is a quest to make the remove callback return void. In the first step of this quest all drivers are converted to .remove_new() which already returns void. Eventually after all drivers are converted, .remove_new() is renamed to .remove().
Trivially convert this driver from always returning zero in the remove callback to the void returning variant.

USB: EHCI: ehci-mv: improve error handling in mv_ehci_enable()
mv_ehci_enable() did not disable and unprepare clocks in case of failures of phy_init(). Besides, it did not take into account failures of ehci_clock_enable() (in effect, failures of clk_prepare_enable()). The patch fixes both issues and gets rid of redundant wrappers around clk_prepare_enable() and clk_disable_unprepare() to simplify this a bit.

The comparison of hcd->irq to less than zero for an error check will never be true because hcd->irq is an unsigned int. Fix this by assigning the int retval to the return of platform_get_irq and checking this for the -ve error condition and assigning hcd->irq to retval.

USB: EHCI: ehci-mv: switch the HSIC HCI to HSIC mode
Turns out the undocumented and reserved bits of port status/control register of the root port need to be set to use the HCI in HSIC mode.
Typically the firmware does this, but that is not always good enough, because the bits get lost if the HSIC clock is disabled (e.g. when ehci-mv is build as a module).
This supplements commit 7b104f890ade ("USB: EHCI: ehci-mv: add HSIC support").

USB: host: ehci: Remove redundant license text
Now that the SPDX tag is in all USB files, that identifies the license in a specific and legally-defined manner. So the extra GPL text wording can be removed as it is no longer needed at all.
This is done on a quest to remove the 700+ different ways that files in the kernel describe the GPL license text. And there's unneeded stuff like the address (sometimes incorrect) for the FSF which is never needed.
No copyright headers or other non-license-description text was removed.

USB: add SPDX identifiers to all remaining files in drivers/usb/
It's good to have SPDX identifiers in all files to make it easier to audit the kernel tree for correct licenses.
Update the drivers/usb/ and include/linux/usb* files with the correct SPDX license identifier based on the license text in the file itself. The SPDX identifier is a legally binding shorthand, which can be used instead of the full boiler plate text.
This work is based on a script and data from Thomas Gleixner, Philippe Ombredanne, and Kate Stewart.

usb: host: ehci-mv: Use devm_ioremap_resource instead of devm_ioremap
Using devm_ioremap_resource() API should actually be preferred over devm_ioremap(), since the former request the mem region first and then gives back the ioremap'ed memory pointer. devm_ioremap_resource() calls request_mem_region(), therby preventing other drivers to make any overlapping call to the same region.
