When write UDC to empty and unbind gadget driver from gadget device, it is possible that there are many queue failures for mass storage function.
The root cause is mass storage main thread alaways try to queue request to receive a command from host if running flag is on, on platform like dwc3, if pull down called, it will not queue request again and return -ESHUTDOWN, but it not affect running flag of mass storage function.
Check return code from mass storage function and clear running flag if it is -ESHUTDOWN, also indicate start in/out transfer failure to break loops.

It allows to reset prevent_medium_removal flag and "eject" the image. This can be useful to free the drive from a hunging host or if the host continues to use the drive even after unmounting (Linux does this). It's also a bit like using an unfolded paperclip on an optical drive.
Previously, the undocumented method of sending SIGUSR1 to a special "file-storage" kernel thread could be used for these purposes, but when using multiple storages there was no way to distinguish one from the other, so we had to send a signal to everyone.

Change the name to reflect this change in functionality. All of the users of complete_and_exit are causing the current kthread to exit so this change makes it clear what is happening.
Move the implementation of kthread_complete_and_exit from kernel/exit.c to to kernel/kthread.c. As this function is kthread specific it makes most sense to live with the kthread functions.
There are no functional change.

For UDCs that support runtime PM, there is a possibility the UDC is already suspended by the time the do_set_interface() is executed. This can lead to HW register access while the UDC is already suspended.

Mac OS-X expects CD-ROM TOC in raw format (i.e. format:2). It also sends the READ_TOC CDB in old style SFF8020i format. i.e. 2 format bits are encoded in MSBs of CDB byte 9.
This patch will enable CD-ROM emulation to work with Mac OS-X. Tested on Mac OS X v10.6.3.
