[SCSI] use scmd_id(), scmd_channel() throughout code
Wrap a highly common idiom. Makes the code easier to read, helps pave the way for sdev->{id,channel} removal, and adds a token that can easily by grepped-for in the future.
There are a couple sdev_id() and scmd_printk() updates thrown in as well.

[SCSI] remove scsi_set_device
scsi_add_host is the proper place to set the device, but people copy the scsi_set_device usage from older drivers again and again.
note that this leaves some legacy drivers like qlogicisp/qlogicfc without pci association in sysfs, but they're scheduled to go away soon anyway.

Linux-2.6.12-rc2
Initial git repository build. I'm not bothering with the full history, even though we have it. We can create a separate "historical" git archive of that later if we want to, and in the meantime it's about 3.2GB when imported into git - space that would just make the early git days unnecessarily complicated, when we don't have a lot of good infrastructure for it.
Let it rip!
