s390: include linux/io.h instead of asm/io.h
Include linux/io.h instead of asm/io.h everywhere. linux/io.h includes asm/io.h, so this shouldn't cause any problems. Instead this might help for some randconfig build errors which were reported due to some undefined io related functions.
Also move the changed include so it stays grouped together with other includes from the same directory.
For ctcm_mpc.c also remove not needed comments (actually questions).

s390/dasd: Establish DMA alignment
linux-next commit bf8d08532bc1 ("iomap: add support for dma aligned direct-io") changes the alignment requirement to come from the block device rather than the block size, and the default alignment requirement is 512-byte boundaries. Since DASD I/O has page alignments for IDAW/TIDAW requests, let's override this value to restore the expected behavior.
Make this change for both ECKD and DIAG disciplines, as they both would fall into this category. Leave FBA alone, since it is always comprised of 512-byte blocks.

Therefore get rid of register asm statement in dasd code, even though there is currently nothing wrong with it. This way we know for sure that the above mentioned bug class won't be introduced here.

s390/dasd: add missing discipline function
Fix crash with illegal operation exception in dasd_device_tasklet. Commit b72949328869 ("s390/dasd: Prepare for additional path event handling") renamed the verify_path function for ECKD but not for FBA and DIAG. This leads to a panic when the path verification function is called for a FBA or DIAG device.
Fix by defining a wrapper function for dasd_generic_verify_path().

s390/dasd: fix inability to use DASD with DIAG driver
During initialization of the DASD DIAG driver a request is issued that has a bio structure that resides on the stack. With virtually mapped kernel stacks this bio address might be in virtual storage which is unsuitable for usage with the diag250 call. In this case the device can not be set online using the DIAG discipline and fails with -EOPNOTSUP. In the system journal the following error message is presented:
dasd: X.X.XXXX Setting the DASD online with discipline DIAG failed with rc=-95
Fix by allocating the bio structure instead of having it on the stack.

s390: Replace zero-length array with flexible-array member
The current codebase makes use of the zero-length array language extension to the C90 standard, but the preferred mechanism to declare variable-length types such as these ones is a flexible array member[1][2], introduced in C99:
struct foo { int stuff; struct boo array[]; };
By making use of the mechanism above, we will get a compiler warning in case the flexible array does not occur last in the structure, which will help us prevent some kind of undefined behavior bugs from being inadvertently introduced[3] to the codebase from now on.
Also, notice that, dynamic memory allocations won't be affected by this change:
"Flexible array members have incomplete type, and so the sizeof operator may not be applied. As a quirk of the original implementation of zero-length arrays, sizeof evaluates to zero."[1]
This issue was found with the help of Coccinelle.

s390: block: add SPDX identifiers to the remaining files
It's good to have SPDX identifiers in all files to make it easier to audit the kernel tree for correct licenses.
Update the drivers/s390/block/ files with the correct SPDX license identifier based on the license text in the file itself. The SPDX identifier is a legally binding shorthand, which can be used instead of the full boiler plate text.
This work is based on a script and data from Thomas Gleixner, Philippe Ombredanne, and Kate Stewart.

s390/dasd: Change unsigned long long to unsigned long
Unsigned long long and unsigned long were different in size for 31-bit. For 64-bit the size for both datatypes is 8 Bytes and since the support for 31-bit is long gone we can clean up a little and change everything to unsigned long. Change get_phys_clock() along the way to accept unsigned long as well so that the DASD code can be consistent.

s390/dasd: fix diag 0x250 inline assembly
git commit 1ec2772e0c3c ("s390/diag: add a statistic for diagnose calls") added function calls to gather diagnose statistics.
In case of the dasd diag driver the function call was added between a register asm statement which initialized register r2 and the inline assembly itself. The function call clobbers the contents of register r2 and therefore the diag 0x250 call behaves in a more or less random way.
Fix this by extracting the function call into a separate function like we do everywhere else.

block: Convert bio_for_each_segment() to bvec_iter
More prep work for immutable biovecs - with immutable bvecs drivers won't be able to use the biovec directly, they'll need to use helpers that take into account bio->bi_iter.bi_bvec_done.
This updates callers for the new usage without changing the implementation yet.

s390/dasd: Implement block timeout handling
This patch implements generic block layer timeout handling callbacks for DASDs. When the timeout expires the respective cqr is aborted.
With this timeout handler time-critical request abort is guaranteed as the abort does not depend on the internal state of the various DASD driver queues.
