mmc_add_host() may return error, if we ignore its return value, the memory that allocated in mmc_alloc_host() will be leaked and it will lead a kernel crash because of deleting not added device in the remove path.
So fix this by checking the return value and goto error path which will call mmc_free_host(), besides, the timer added before mmc_add_host() needs be del.
And this patch fixes another missing call mmc_free_host() if usb_control_msg() fails.

The current codebase makes use of the zero-length array language extension to the C90 standard, but the preferred mechanism to declare variable-length types such as these ones is a flexible array member[1][2], introduced in C99:
struct foo { int stuff; struct boo array[]; };
By making use of the mechanism above, we will get a compiler warning in case the flexible array does not occur last in the structure, which will help us prevent some kind of undefined behavior bugs from being inadvertently introduced[3] to the codebase from now on.
Also, notice that, dynamic memory allocations won't be affected by this change:
"Flexible array members have incomplete type, and so the sizeof operator may not be applied. As a quirk of the original implementation of zero-length arrays, sizeof evaluates to zero."[1]
This issue was found with the help of Coccinelle.

mmc: Convert timers to use timer_setup()
In preparation for unconditionally passing the struct timer_list pointer to all timer callbacks, switch to using the new timer_setup() and from_timer() to pass the timer pointer explicitly.

The mmc_card_present() function helps the mmc core to track an internal state of the card device. More importantly, it's not intended to be used by mmc host drivers to check for an inserted card. Therefore, let's stop using it and instead rely on checking for a valid pointer to a struct mmc_card, as it should be good enough.

Suspend and resume of cards are handled by the protocol layer and consequently the mmc_suspend|resume_host APIs are marked as deprecated.

This means we can simplify the suspend|resume callbacks by removing the use of the deprecated APIs.
