Commit a4fdd9762272 ("iommu: Use flush queue capability") hid the
IOMMU_DOMAIN_DMA_FQ domain type from domain allocation. A check was
introduced in iommu_dma_init_domain() to fall back if not supported, but
this check runs too late: by that point, devices have been attached to
the IOMMU, and apple-dart's attach_dev() callback does not expect
IOMMU_DOMAIN_DMA_FQ domains.

Change the logic so the IOMMU_DOMAIN_DMA codepath is the default,
instead of explicitly enumerating all types.

Fixes an apple-dart regression in v6.5.

The .remove() callback for a platform driver returns an int which makes
many driver authors wrongly assume it's possible to do error handling by
returning an error code. However the value returned is (mostly) ignored
and this typically results in resource leaks. To improve here there is a
quest to make the remove callback return void. In the first step of this
quest all drivers are converted to .remove_new() which already returns
void.

Trivially convert this driver from always returning zero in the remove
callback to the void returning variant.

pci_device_group() can return an already existing IOMMU group if the PCI
device's pagetables have to be shared with another one due to bus
toplogy, isolation features and/or DMA alias quirks.
apple_dart_device_group() however assumes that the group has just been
created and overwrites its iommudata which will eventually lead to
apple_dart_release_group leaving stale entries in sid2group.
Fix that by merging the iommudata if the returned group already exists.

The iommu core calls the driver's detach_dev domain op callback only when
a device is finished assigning to user space and
iommu_group_release_dma_owner() is called to return the device to the
kernel, where iommu core wants to set the default domain to the device but
the driver didn't provide one.

In other words, if any iommu driver provides default domain support, the
.detach_dev callback will never be called. This removes the detach_dev
callbacks in those IOMMU drivers that support default domain.

DART has an additional global register to control which streams are
isolated. This register is a bit redundant since DART_TCR can already
be used to control isolation and is usually initialized to DART_STREAM_ALL
by the time we get control. Some DARTs (namely the one used for the audio
controller) however have some streams disabled initially. Make sure those
work by initializing DART_STREAMS_ENABLE during reset.
