drm/i915: Only include i915_reg.h from .c files

Several of our i915 header files, have been including i915_reg.h. This
means that any change to i915_reg.h will trigger a full rebuild of
pretty much every file of the driver, even those that don't have any
kind of register access. Let's delete the i915_reg.h include from all
headers and add an explicit include from the .c files that truly
need the register definitions; those that need a definition of
i915_reg_t for a function definition can get it from i915_reg_defs.h
instead.

We also remove two non-register #define's (VLV_DISPLAY_BASE and
GEN12_SFC_DONE_MAX) into i915_reg_defs.h to allow us to drop the
i915_reg.h include from a couple of headers.

There's probably a lot more header dependency optimization possible, but
the changes here roughly cut the number of files compiled after 'touch
i915_reg.h' in half --- a good first step.

drm/i915/gt: Move engine registers to their own header

Let's continue breaking up and cleaning up the massive i915_reg.h file
by moving all registers that are defined in relation to an engine base
to their own header.

There are probably a bunch of other "engine registers" that we haven't
moved yet (especially those that belong to the render engine in the
0x2??? range), but this is a relatively straightforward first step.

drm/i915/gvt: Fix incorrect check of enabled bits in mask registers

Using _MASKED_BIT_ENABLE macro to set mask register bits is straight
forward and not likely to go wrong. However when checking which bit(s)
is(are) enabled, simply bitwise AND value and _MASKED_BIT_ENABLE() won't
output expected result. Suppose the register write is disabling bit 1
by setting 0xFFFF0000, however "& _MASKED_BIT_ENABLE(1)" outputs
0x00010000, and the non-zero check will pass which cause the old code
consider the new value set as an enabling operation.

We found guest set 0x80008000 on boot, and set 0xffff8000 during resume.
Both are legal settings but old code will block latter and force vgpu
enter fail-safe mode.

Introduce two new macro and make proper masked bit check in mmio handler:
IS_MASKED_BITS_ENABLED()
IS_MASKED_BITS_DISABLED()

drm/i915/gvt: init mmio by lri command in vgpu inhibit context

There is one issue relates to Coarse Power Gating(CPG) on KBL NUC in GVT-g,
vgpu can't get the correct default context by updating the registers before
inhibit context submission. It always get back the hardware default value
unless the inhibit context submission happened before the 1st time
forcewake put. With this wrong default context, vgpu will run with
incorrect state and meet unknown issues.

The solution is initialize these mmios by adding lri command in ring buffer
of the inhibit context, then gpu hardware has no chance to go down RC6 when
lri commands are right being executed, and then vgpu can get correct
default context for further use.

drm/i915/gvt: Fix incorrect check of enabled bits in mask registers

IS_MASKED_BITS_ENABLED()
IS_MASKED_BITS_DISABLED()

V2: Rebase.

drm/i915: Store a pointer to intel_context in i915_request

To ease the frequent and ugly pointer dance of
&request->gem_context->engine[request->engine->id] during request
submission, store that pointer as request->hw_context. One major
advantage that we will exploit later is that this decouples the logical
context state from the engine itself.

drm/i915/gvt: init mmio by lri command in vgpu inhibit context
