When alloc_scq fails, card->vcs[0] (i.e. vc) should be freed. Otherwise, in the following call chain:
idt77252_init_one |-> idt77252_dev_open |-> open_card_ubr0 |-> alloc_scq [failed] |-> deinit_card |-> vfree(card->vcs);
card->vcs is freed and card->vcs[0] is leaked.

The root cause is traced to the vc_maps which alloced in open_card_oam() are not freed in close_card_oam(). The vc_maps are used to record open connections, so when close a vc_map in close_card_oam(), the memory should be freed. Moreover, the ubr0 is not closed when close a idt77252 device, leading to the memory leak of vc_map and scq_info.
Fix them by adding kfree in close_card_oam() and implementing new close_card_ubr0() to close ubr0.

Due to several bugs caused by timers being re-armed after they are shutdown and just before they are freed, a new state of timers was added called
treewide: Convert del_timer*() to timer_shutdown*()
Due to several bugs caused by timers being re-armed after they are shutdown and just before they are freed, a new state of timers was added called "shutdown". After a timer is set to this state, then it can no longer be re-armed.
The following script was run to find all the trivial locations where del_timer() or del_timer_sync() is called in the same function that the object holding the timer is freed. It also ignores any locations where the timer->function is modified between the del_timer*() and the free(), as that is not considered a "trivial" case.
This was created by using a coccinelle script and the following commands:

Then skb->data is accessed on lines 862 and 863: tbd->word_4 = (skb->data[0] << 24) | (skb->data[1] << 16) | (skb->data[2] << 8) | (skb->data[3] << 0); and on lines 893 and 894: tbd->word_4 = (skb->data[0] << 24) | (skb->data[1] << 16) | (skb->data[2] << 8) | (skb->data[3] << 0);
These accesses may cause data inconsistency between CPU cache and hardware.
To fix this problem, the calculation result of skb->data is stored in a local variable before DMA mapping, and then the driver accesses this local variable instead of skb->data.

This converts all remaining cases of the old setup_timer() API into using timer_setup(), where the callback argument is the structure already holding the struct timer_list. These should have no behavioral changes, since they just change which pointer is passed into the callback with the same available pointers after conversion. It handles the following examples, in addition to some other variations.
Casting from unsigned long:
void my_callback(unsigned long data) { struct something *ptr = (struct something *)data; ... } ... setup_timer(&ptr->my_timer, my_callback, ptr);
and forced object casts:
void my_callback(struct something *ptr) { ... } ... setup_timer(&ptr->my_timer, my_callback, (unsigned long)ptr);

In preparation for unconditionally passing the struct timer_list pointer to all timer callbacks, switch to using the new timer_setup() and from_timer() to pass the timer pointer explicitly. This required adding a pointer back to vc_map, and adjusting the locking around removal a bit.

Make these structures const as they are either passed to the function atm_dev_register having the corresponding argument as const or stored in the ops field of a atm_dev structure, which is also const. Done using Coccinelle.
