atm: eni: Use tasklet_disable_in_atomic() in the send() callback

The atmdev_ops::send callback which calls tasklet_disable() is invoked with
bottom halfs disabled from net_device_ops::ndo_start_xmit(). All other
invocations of tasklet_disable() in this driver happen in preemptible
context.

Change the send() call to use tasklet_disable_in_atomic() which allows
tasklet_disable() to be made sleepable once the remaining atomic context
usage sites are cleaned up.

atm: eni: dont release is never initialized

label err_eni_release is reachable when eni_start() fail.
In eni_start() it calls dev->phy->start() in the last step, if start()
fail we don't need to call phy->stop(), if start() is never called, we
neither need to call phy->stop(), otherwise null-ptr-deref will happen.

In order to fix this issue, don't call phy->stop() in label err_eni_release

atm: eni: avoid accessing the data mapped to streaming DMA

In do_tx(), skb->data is mapped to streaming DMA on line 1111:
paddr = dma_map_single(...,skb->data,DMA_TO_DEVICE);

Then skb->data is accessed on line 1153:
(skb->data[3] & 0xf)

This access may cause data inconsistency between CPU cache and hardware.

To fix this problem, skb->data[3] is assigned to a local variable before
DMA mapping, and then the driver accesses this local variable instead of
skb->data[3].

lib/vsprintf.c: remove %Z support

Now that %z is standartised in C99 there is no reason to support %Z.
Unlike %L it doesn't even make format strings smaller.

Use BUILD_BUG_ON in a couple ATM drivers.

In case anyone didn't notice lib/vsprintf.o is about half of SLUB which
is in my opinion is quite an achievement. Hopefully this patch inspires
someone else to trim vsprintf.c more.

hlist: drop the node parameter from iterators

I'm not sure why, but the hlist for each entry iterators were conceived

list_for_each_entry(pos, head, member)

The hlist ones were greedy and wanted an extra parameter:

hlist_for_each_entry(tpos, pos, head, member)

Why did they need an extra pos parameter? I'm not quite sure. Not only
they don't really need it, it also prevents the iterator from looking
exactly like the list iterator, which is unfortunate.

Besides the semantic patch, there was some manual work required:

- Fix up the actual hlist iterators in linux/list.h
- Fix up the declaration of other iterators based on the hlist ones.
- A very small amount of places were using the 'node' parameter, this
was modified to use 'obj->member' instead.
- Coccinelle didn't handle the hlist_for_each_entry_safe iterator
properly, so those had to be fixed up manually.

The semantic patch which is mostly the work of Peter Senna Tschudin is here:

Drivers: atm: remove __dev* attributes.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.

This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.

Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.
