Functions coherent_kvaddr, clear_page_alias and copy_page_alias use physical address 0 as a special value that means 'this page is in the KSEG mapping and its existing virtual address has the same color as the virtual address of its future mapping, so don't map it to the TLBTEMP_BASE area'.
Simplify this logic and drop special handling of low memory pages/pages with coherent mapping and always use TLBTEMP_BASE area.

Disable preemption around all cache alias management calls to enforce that.

Patch series "mm: cleanup usage of <asm/pgalloc.h>"
Most architectures have very similar versions of pXd_alloc_one() and pXd_free_one() for intermediate levels of page table. These patches add generic versions of these functions in <asm-generic/pgalloc.h> and enable use of the generic functions where appropriate.
In addition, functions declared and defined in <asm/pgalloc.h> headers are used mostly by core mm and early mm initialization in arch and there is no actual reason to have the <asm/pgalloc.h> included all over the place. The first patch in this series removes unneeded includes of <asm/pgalloc.h>
In the end it didn't work out as neatly as I hoped and moving pXd_alloc_track() definitions to <asm-generic/pgalloc.h> would require unnecessary changes to arches that have custom page table allocations, so I've decided to move lib/ioremap.c to mm/ and make pgalloc-track.h local to mm/.
This patch (of 8):
In most cases <asm/pgalloc.h> header is required only for allocations of page table memory. Most of the .c files that include that header do not use symbols declared in <asm/pgalloc.h> and do not require that header.
As for the other header files that used to include <asm/pgalloc.h>, it is possible to move that include into the .c file that actually uses symbols from <asm/pgalloc.h> and drop the include from the header file.
The process was somewhat automated using
sed -i -E '/[<"]asm\/pgalloc\.h/d' \ $(grep -L -w -f /tmp/xx \ $(git grep -E -l '[<"]asm/pgalloc\.h'))
where /tmp/xx contains all the symbols defined in arch/*/include/asm/pgalloc.h.

Thanks to commit 4b3ef9daa4fc ("mm/swap: split swap cache into 64MB trunks"), after swapoff the address_space associated with the swap device will be freed. So page_mapping() users which may touch the address_space need some kind of mechanism to prevent the address_space from being freed during accessing.
The dcache flushing functions (flush_dcache_page(), etc) in architecture specific code may access the address_space of swap device for anonymous pages in swap cache via page_mapping() function. But in some cases there are no mechanisms to prevent the swap device from being swapoff, for example,
CPU1 CPU2 __get_user_pages() swapoff() flush_dcache_page() mapping = page_mapping() ... exit_swap_address_space() ... kvfree(spaces) mapping_mapped(mapping)
The address space may be accessed after being freed.
But from cachetlb.txt and Russell King, flush_dcache_page() only care about file cache pages, for anonymous pages, flush_anon_page() should be used. The implementation of flush_dcache_page() in all architectures follows this too. They will check whether page_mapping() is NULL and whether mapping_mapped() is true to determine whether to flush the dcache immediately. And they will use interval tree (mapping->i_mmap) to find all user space mappings. While mapping_mapped() and mapping->i_mmap isn't used by anonymous pages in swap cache at all.
So, to fix the race between swapoff and flush dcache, __page_mapping() is add to return the address_space for file cache pages and NULL otherwise. All page_mapping() invoking in flush dcache functions are replaced with page_mapping_file().

This happens because implementation of these functions is only compiled when data cache is WB, which looks wrong: even when data cache doesn't need flushing it still needs invalidation. The functions like __flush_[invalidate_]dcache_* are correctly defined for both WB and WT caches (and even if they weren't that'd still be ok, just slower).
Fix this by providing the same implementation of the above functions for both WB and WT cache.

Existing clear_user_page and copy_user_page cannot be used with highmem because they calculate physical page address from its virtual address and do it incorrectly in case of high memory page mapped with kmap_atomic. Also kmap is not needed, as most likely userspace mapping color would be different from the kmapped color.
Provide clear_user_highpage and copy_user_highpage functions that determine if temporary mapping is needed for the pages. Move most of the logic of the former clear_user_page and copy_user_page to xtensa/mm/cache.c only leaving temporary mapping setup, invalidation and clearing/copying in the xtensa/mm/misc.S. Rename these functions to clear_page_alias and copy_page_alias.

On VIVT ARM, when we have multiple shared mappings of the same file in the same MM, we need to ensure that we have coherency across all copies. We do this via make_coherent() by making the pages uncacheable.
This used to work fine, until we allowed highmem with highpte - we now have a page table which is mapped as required, and is not available for modification via update_mmu_cache().
Ralf Beache suggested getting rid of the PTE value passed to update_mmu_cache():
On MIPS update_mmu_cache() calls __update_tlb() which walks pagetables to construct a pointer to the pte again. Passing a pte_t * is much more elegant. Maybe we might even replace the pte argument with the pte_t?
Ben Herrenschmidt would also like the pte pointer for PowerPC:
Passing the ptep in there is exactly what I want. I want that -instead- of the PTE value, because I have issue on some ppc cases, for I$/D$ coherency, where set_pte_at() may decide to mask out the _PAGE_EXEC.
So, pass in the mapped page table pointer into update_mmu_cache(), and remove the PTE value, updating all implementations and call sites to suit.

Cache-aliasing means that the size of the cache (for one way) is larger than the page size, thus, a page can end up in several places in cache depending on the virtual to physical translation. The method used here is to map a user page temporarily through the auto-refill way 0 and of of the DTLB. We probably will want to revisit this issue and use a better approach with kmap/kunmap.

These patches add generic versions of these functions in <asm-generic/pgalloc.h> and enable use of the generic functions where appropriate. In addition, functions declared and defined in <asm/pgalloc.h> headers are used mostly by core mm and early mm initialization in arch and there is no actual reason to have the <asm/pgalloc.h> included all over the place. The first patch in this series removes unneeded includes of <asm/pgalloc.h> In the end it didn't work out as neatly as I hoped and moving pXd_alloc_track() definitions to <asm-generic/pgalloc.h> would require unnecessary changes to arches that have custom page table allocations, so I've decided to move lib/ioremap.c to mm/ and make pgalloc-track.h local to mm/. This patch (of 8): In most cases <asm/pgalloc.h> header is required only for allocations of page table memory. Most of the .c files that include that header do not use symbols declared in <asm/pgalloc.h> and do not require that header.
