mm: Rename arch pte_mkwrite()'s to pte_mkwrite_novma()
The x86 Shadow stack feature includes a new type of memory called shadow stack. This shadow stack memory has some unusual properties, which requires some core mm changes to function properly.
One of these unusual properties is that shadow stack memory is writable, but only in limited ways. These limits are applied via a specific PTE bit combination. Nevertheless, the memory is writable, and core mm code will need to apply the writable permissions in the typical paths that call pte_mkwrite(). The goal is to make pte_mkwrite() take a VMA, so that the x86 implementation of it can know whether to create regular writable or shadow stack mappings.
But there are a couple of challenges to this. Modifying the signatures of each arch pte_mkwrite() implementation would be error prone because some are generated with macros and would need to be re-implemented. Also, some pte_mkwrite() callers operate on kernel memory without a VMA.
So this can be done in a three step process. First pte_mkwrite() can be renamed to pte_mkwrite_novma() in each arch, with a generic pte_mkwrite() added that just calls pte_mkwrite_novma(). Next callers without a VMA can be moved to pte_mkwrite_novma(). And lastly, pte_mkwrite() and all callers can be changed to take/pass a VMA.
Start the process by renaming pte_mkwrite() to pte_mkwrite_novma() and adding the pte_mkwrite() wrapper in linux/pgtable.h. Apply the same pattern for pmd_mkwrite(). Since not all archs have a pmd_mkwrite_novma(), create a new arch config HAS_HUGE_PAGE that can be used to tell if pmd_mkwrite() should be defined. Otherwise in the !HAS_HUGE_PAGE cases the compiler would not be able to find pmd_mkwrite_novma().
No functional change.

Let's support __HAVE_ARCH_PTE_SWP_EXCLUSIVE by stealing one bit from the type. Generic MM currently only uses 5 bits for the type (MAX_SWAPFILES_SHIFT), so the stolen bit is effectively unused.
The shift by 2 when converting between PTE and arch-specific swap entry makes the swap PTE layout a little bit harder to decipher.
While at it, drop the comment from paulus---copy-and-paste leftover from powerpc where we actually have _PAGE_HASHPTE---and mask the type in __swp_entry_to_pte() as well.

The microblaze's implementation of pte_alloc_one_kernel() used memblock_alloc_try_nid_raw() along with clear_page() to allocated a zeroed page during early setup.
Replace calls of these functions with a call to memblock_alloc_try_nid() that already returns zeroed page and respects the same allocation limits as memblock_alloc_try_nid_raw().
While on it drop early_get_page() wrapper that was only used in pte_alloc_one_kernel().

The default FIRST_USER_ADDRESS here would be skipped in <linux/pgtable.h> when the given platform overrides its value via <asm/pgtable.h>.

For the most architectures the pte_offset_kernel() implementation relies on the availability of pmd_page_vaddr() that converts a PMD entry value to the virtual address of the page containing PTEs array.
Let's move x86 definitions of the PTE accessors to the generic place in <linux/pgtable.h> and then simply drop the respective definitions from the other architectures.
The architectures that didn't provide pmd_page_vaddr() are updated to have that defined.
The generic implementation of pte_offset_kernel() can be overridden by an architecture and alpha makes use of this because it has special ordering requirements for its version of pte_offset_kernel().

Currently there are many platforms that dont enable ARCH_HAS_PTE_SPECIAL but required to define quite similar fallback stubs for special page table entry helpers such as pte_special() and pte_mkspecial(), as they get build in generic MM without a config check. This creates two generic fallback stub definitions for these helpers, eliminating much code duplication.
mips platform has a special case where pte_special() and pte_mkspecial() visibility is wider than what ARCH_HAS_PTE_SPECIAL enablement requires. This restricts those symbol visibility in order to avoid redefinitions which is now exposed through this new generic stubs and subsequent build failure. arm platform set_pte_at() definition needs to be moved into a C file just to prevent a build failure.

Both pgtable_cache_init() and pgd_cache_init() are used to initialize kmem cache for page table allocations on several architectures that do not use PAGE_SIZE tables for one or more levels of the page table hierarchy.
Most architectures do not implement these functions and use __weak default NOP implementation of pgd_cache_init(). Since there is no such default for pgtable_cache_init(), its empty stub is duplicated among most architectures.
Rename the definitions of pgd_cache_init() to pgtable_cache_init() and drop empty stubs of pgtable_cache_init().

The only functional differences (modulo a few missing fixes in the arch code) is that architectures without coherent caches need a hook to convert a virtual or dma address into a pfn, given that we don't have the kernel linear mapping available for the otherwise easy virt_to_page call. As a side effect we can support mmap of the per-device coherent area even on architectures not providing the callback, and we make previous dangerous default methods dma_common_mmap actually save for non-coherent architectures by rejecting it without the right helper.
In addition to that we need a hook so that some architectures can override the protection bits when mmaping a dma coherent allocations.

In this, on tile, we have FIRST_USER_ADDRESS defined as 0. round_up() has the same type -- int. PUD_SHIFT.
I think the best way to fix it is to define FIRST_USER_ADDRESS as unsigned long. On every arch for consistency.

Defining __PAGETABLE_PMD_FOLDED will drop out unused __pmd_alloc(). It also fixes problems with recently-introduced pmd accounting.
