The numa_slit variable used by node_distance is available to a module as long as it is linked at compile-time. However, it is not available to loadable modules. Leading to errors such as:
ERROR: "numa_slit" [drivers/nvme/host/nvme-core.ko] undefined!
The error above is caused by the nvme multipath code that makes use of node_distance for its path calculation. When the patch was added, the lightnvm subsystem would select nvme and always compile it in, leading to the node_distance call to always succeed. However, when this requirement was removed, nvme could be compiled in as a module, which exposed this bug.
This patch extracts node_distance to a function and exports it. Since ACPI is depending on node_distance being a simple lookup to numa_slit, the previous behavior is exposed as slit_distance and its users updated.

acpi_map_pxm_to_node() unconditially maps nodes even when NUMA is turned off. So acpi_get_node() might return a node > 0, which is fatal when NUMA is disabled as the rest of the kernel assumes that only node 0 exists.
Expose numa_off to the acpi code and return NUMA_NO_NODE when it's set.

In arch ia64, has following definition: extern u8 numa_slit[MAX_NUMNODES * MAX_NUMNODES]; #define node_distance(from,to) (numa_slit[(from) * num_online_nodes() + (to)])
num_online_nodes() is a variable value, it can be changed after hot-remove/add a node.
In my practice, I found node distance is wrong after offline a node in IA64 platform. For example system has 4 nodes: node distances: node 0 1 2 3 0: 10 21 21 32 1: 21 10 32 21 2: 21 32 10 21 3: 32 21 21 10

Move definition of NUMA_NO_NODE from ia64 and x86_64 arch specific headers to generic header 'linux/numa.h' for use in generic code. NUMA_NO_NODE replaces bare '-1' where it's used in this series to indicate "no node id specified". Ultimately, it can be used to replace the -1 elsewhere where it is used similarly.

After moving the the include files there were a few clean-ups:
1) Some files used #include <asm-ia64/xyz.h>, changed to <asm/xyz.h>
2) Some comments alerted maintainers to look at various header files to make matching updates if certain code were to be changed. Updated these comments to use the new include paths.
3) Some header files mentioned their own names in initial comments. Just deleted these self references.

ia64: export node_distance function The numa_slit variable used by node_distance is available to a module as long as it is linked at compile-time. However, it is not available to loadable modules. Leading to errors such as: ERROR: "numa_slit" [drivers/nvme/host/nvme-core.ko] undefined! The error above is caused by the nvme multipath code that makes use of node_distance for its path calculation. When the patch was added, the lightnvm subsystem would select nvme and always compile it in, leading to the node_distance call to always succeed. However, when this requirement was removed, nvme could be compiled in as a module, which exposed this bug. This patch extracts node_distance to a function and exports it. Since ACPI is depending on node_distance being a simple lookup to numa_slit, the previous behavior is exposed as slit_distance and its users updated. So acpi_get_node() might return a node > 0, which is fatal when NUMA is disabled as the rest of the kernel assumes that only node 0 exists. Expose numa_off to the acpi code and return NUMA_NO_NODE when it's set. [IA64] Fix a node distance bug In arch ia64, has following definition: extern u8 numa_slit[MAX_NUMNODES * MAX_NUMNODES]; #define node_distance(from,to) (numa_slit[(from) * num_online_nodes() + (to)]) num_online_nodes() is a variable value, it can be changed after hot-remove/add a node. In my practice, I found node distance is wrong after offline a node in IA64 platform. NUMA_NO_NODE replaces bare '-1' where it's used in this series to indicate "no node id specified". Ultimately, it can be used to replace the -1 elsewhere where it is used similarly. [IA64] Move include/asm-ia64 to arch/ia64/include/asm After moving the the include files there were a few clean-ups: 1) Some files used #include <asm-ia64/xyz.h>, changed to <asm/xyz.h> 2) Some comments alerted maintainers to look at various header files to make matching updates if certain code were to be changed. Updated these comments to use the new include paths. 3) Some header files mentioned their own names in initial comments. Just deleted these self references.
