net_sched: use void pointer for filter handle

Now we use 'unsigned long fh' as a pointer in every place,
it is safe to convert it to a void pointer now. This gets
rid of many casts to pointer.

This patchset is one of the couple cleanup patchsets I have in queue.
The motivation aside the obvious need to "make things nicer" is also
to prepare for shared filter blocks introduction. That requires tp->q
removal, and therefore removal of all tp->q users.

Patch 1 is just some small thing I spotted on the way
Patch 2 removes one user of tp->q, namely tcf_em_tree_change
Patches 3-8 do preparations for exts->nr_actions removal
Patches 9-10 do simple renames of functions in cls*
Patches 11-19 remove unnecessary calls of tcf_exts_change helper
The last patch changes tcf_exts_change to don't take lock

Tested by tools/testing/selftests/tc-testing

v1->v2:
- removed conversion of action array to list as noted by Cong
- added the past patch instead
- small rebases of patches 11-19

net_sched: move the empty tp check from ->destroy() to ->delete()

We could have a race condition where in ->classify() path we
dereference tp->root and meanwhile a parallel ->destroy() makes it
a NULL. Daniel cured this bug in commit d936377414fa
("net, sched: respect rcu grace period on cls destruction").

This happens when ->destroy() is called for deleting a filter to
check if we are the last one in tp, this tp is still linked and
visible at that time. The root cause of this problem is the semantic
of ->destroy(), it does two things (for non-force case):

1) check if tp is empty
2) if tp is empty we could really destroy it

and its caller, if cares, needs to check its return value to see if it
is really destroyed. Therefore we can't unlink tp unless we know it is
empty.

As suggested by Daniel, we could actually move the test logic to ->delete()
so that we can safely unlink tp after ->delete() tells us the last one is
just deleted and before ->destroy().
