Use ODP MRs for kernel ULPs

The following series extends MR creation routines to allow creation of
user MRs through kernel ULPs as a proxy. The immediate use case is to
allow RDS to work over FS-DAX, which requires ODP (on-demand-paging)
MRs to be created and such MRs were not possible to create prior this
series.

The first part of this patchset extends RDMA to have special verb
ib_reg_user_mr(). The common use case that uses this function is a
userspace application that allocates memory for HCA access but the
responsibility to register the memory at the HCA is on an kernel ULP.
This ULP acts as an agent for the userspace application.

The second part provides advise MR functionality for ULPs. This is
integral part of ODP flows and used to trigger pagefaults in advance
to prepare memory before running working set.

The third part is actual user of those in-kernel APIs.

The open method of hiddev handler fails to bring the device out of
autosuspend state as was promised in 0361a28d3f9a, as it actually has 2
blocks that try to start the transport (call hid_hw_open()) with both
being guarded by the "open" counter, so the 2nd block is never executed as
the first block increments the counter so it is never at 0 when we check
it for the second block.

Additionally hiddev_open() was leaving counter incremented on errors,
causing the device to never be reopened properly if there was ever an
error.

Let's fix all of this by factoring out code that creates client structure
and powers up the device into a separate function that is being called
from usbhid_open() with the "existancelock" being held.

fs/compat_ioctl.c, which still has a couple of commands that need
support for time64_t.

In completely unrelated work, I spent time on cleaning up parts of
this file in the past, moving things out into drivers instead.

After Al Viro reviewed an earlier version of this series and did a lot
more of that cleanup, I decided to try to completely eliminate the
rest of it and move it all into drivers.

This series incorporates some of Al's work and many patches of my own,
but in the end stops short of actually removing the last part, which
is the scsi ioctl handlers. I have patches for those as well, but they
need more testing or possibly a rewrite"
