spi: Replace all spi->chip_select and spi->cs_gpiod references with function call
Supporting multi-cs in spi drivers would require the chip_select & cs_gpiod members of struct spi_device to be an array. But changing the type of these members to array would break the spi driver functionality. To make the transition smoother introduced four new APIs to get/set the spi->chip_select & spi->cs_gpiod and replaced all spi->chip_select and spi->cs_gpiod references with get or set API calls. While adding multi-cs support in further patches the chip_select & cs_gpiod members of the spi_device structure would be converted to arrays & the "idx" parameter of the APIs would be used as array index i.e., spi->chip_select[idx] & spi->cs_gpiod[idx] respectively.

The .remove() callback for a platform driver returns an int which makes many driver authors wrongly assume it's possible to do error handling by returning an error code. However the value returned is (mostly) ignored and this typically results in resource leaks. To improve here there is a quest to make the remove callback return void. In the first step of this quest all drivers are converted to .remove_new() which already returns void.
Trivially convert this driver from always returning zero in the remove callback to the void returning variant.

NO_IRQ is used to check the return of irq_of_parse_and_map().
On some architecture NO_IRQ is 0, on other architectures it is -1.
irq_of_parse_and_map() returns 0

In a recent change to the SPI subsystem [1], a new `delay` struct was added to replace the `delay_usecs`. This change replaces the current `delay_usecs` with `delay` for this driver.
The `spi_transfer_delay_exec()` function [in the SPI framework] makes sure that both `delay_usecs` & `delay` are used (in this order to preserve backwards compatibility).

This issue was detected by using the Coccinelle software.
* Replace the specification of a data type by a pointer dereference to make the corresponding size determination a bit safer according to the Linux coding style convention.

CONFIG_HOTPLUG is going away as an option. As result the __dev* markings will be going away.
Remove use of __devinit, __devexit_p, __devinitdata, __devinitconst, and __devexit.
Bill Pemberton has done most of the legwork on this series. I've used his script to purge the attributes from the drivers/gpio tree.

The call sequence spi_alloc_master/spi_register_master/spi_unregister_master is complete; it reduces the device reference count to zero, which results in device memory being freed. An extra call to spi_master_put is unnecessary and results in an access to free memory.
At the same time, since the call to spi_unregister_master results in device memory being freed, it must no longer be accessed afterwards.
To fix both problems, call spi_master_get to get an extra reference to the device, and call spi_master_put only after the last access to device data.

Instead of requiring spi master drivers to always set the bus number (even when a dynamic number is desired), this patch modifies spi_alloc_master() to initialize num_chipselect to 1 (because there will always be at least one CS) and bus_num to -1 for dynamic allocation. This simplifies the code needed to be written for drivers.
